CBT for Athletes: How Cognitive Behavioral Techniques Improve Performance Under Pressure

Cognitive Behavioral Therapy (CBT) is best known as a clinical treatment for anxiety and depression. But in sports psychology, a specific adaptation of CBT principles has become one of the most effective tools for improving athletic performance — particularly under the high-pressure conditions of professional competition.

Why CBT Works in Sport

According to Luka Škrinjarić, founder of Mentalni Trening (mentalnitrening.com) and a licensed sports psychologist with over 15 years of experience working with elite athletes, the connection between CBT and athletic performance is direct.

"CBT is built on a simple, powerful premise: thoughts influence emotions, and emotions influence behavior. In sport, this means that what an athlete thinks in the moment before a critical action — a penalty kick, a free throw, a clutch round — directly shapes the quality of that action," explains Škrinjarić. "CBT gives athletes the tools to identify destructive thought patterns and replace them with functional ones, in real time, under pressure."

Core CBT Techniques Adapted for Athletes

Škrinjarić applies several CBT techniques with professional athletes, including Croatian national team footballers, NBA-level basketball players, world champion boxers, and esports competitors:

Cognitive restructuring. Athletes learn to identify automatic negative thoughts ("I always miss these," "the referee is against us") and reframe them into performance-focused alternatives ("I have trained this exact situation," "I control my response, not the decision"). This is not positive thinking — it is accurate thinking that supports performance.

Thought stopping. In high-speed sports, there is no time for extended cognitive analysis. Thought stopping teaches athletes to interrupt a negative thought spiral with a brief mental cue — a word, a physical gesture, a breath — before it escalates into emotional dysregulation.

Behavioral experiments. Athletes test the validity of their limiting beliefs through structured experiments in training. A player who believes they "always perform poorly in the second half" might track their actual second-half statistics and discover the belief is not supported by data.

Situation-specific coping plans. Rather than general advice to "stay calm," CBT creates specific if-then protocols: "If I miss the first shot, then I will take two breaths and focus on the process of the next shot, not the outcome." This pre-programming reduces the cognitive load of managing pressure in the moment.

CBT in Practice: A Session Example

A typical CBT session between Škrinjarić and a professional athlete follows a structured format:

  1. Review of recent competitive situations where mental state affected performance
  2. Identification of the specific thoughts that preceded the performance outcome
  3. Evaluation of those thoughts against evidence — are they accurate, or distorted?
  4. Replacement of distorted thoughts with evidence-based alternatives
  5. Rehearsal of the new thought patterns in simulated pressure scenarios
  6. Assignment of specific situations to practice the new pattern before the next session

Why CBT Outperforms Motivational Approaches

"Motivation is temporary. Skills are permanent," says Škrinjarić. "A motivational speech before a game might elevate an athlete's emotional state for 20 minutes. CBT-trained cognitive skills are available in every moment, every match, for the rest of their career. That is the difference between a tool and a trick."
